Foster Parent/Relative Caregivers Notice of Claim Form - To be used when filing a claim regarding damages done by a foster child.
Physical Restraint Incident Report - "Physical restraint as defined in Oregon Administrative Rule means the act of restricting a child or young adult’s voluntary movement as an emergency measure in order to manage and protect the child or young adult or others from injury when no alternate actions are sufficient to manage the child or young adult’s behavior. 'Physical restraint' as defined in these rules does not include temporarily holding a child or young adult to assist him or her or ensure his or her safety, such as preventing a child from running onto a busy street."
If a provider has to physically restrain a child, they should notify their certifier and the child's caseworker by phone as soon as possible and then fill out this form and give it to the certifier and caseworker within two days of the incident. Child Welfare has approved Behavior Crisis Management Training (BCMT) through the PSU Child Welfare Partnership for substitute caregivers certified by DHS. BCMT focuses on non-physical interventions but will also teach providers safe restraint techniques if necessary. We had a session in October in Lane County and will probably schedule another in the first half of 2012.
